State of the Heart Hospice serves our communities by providing individualized, exceptional care to patients and families experiencing a life-limiting illness, enhancing their quality of life. • Full Time STNA – Completion of State approved 75-hour formal training program and experience required
• Full Time RN – Two years of experience in Medical/Surgical, oncology or community health preferred
• Nurse Practitioner – Minimum of five years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care
• Patient Care Liaison/LPN – Two years of experience in Medical/Surgical, oncology or community health preferred
